Athens Daily Connect is a daily wellness check-in service designed to provide an extra layer of connection and support for adults living independently.
A Friendly Daily Call
At the participant’s chosen time, Athens Daily Connect places a brief, friendly check-in call to see how they’re doing.
A Chance to Check In
The call gives participants an easy way to share how they’re doing, mention a concern, or request contact from Athens Home Care Solutions.
Follow-Up When Needed
If something comes up during the call that may need attention, Athens Home Care Solutions follows the participant’s individualized response plan.
How It Works
Simple, consistent check-ins—with a plan for what happens next.
Choose a Check-in Time
Participants choose a preferred daily call time during available calling hours.
Receive the Daily Call
Athens Daily Connect places a brief, friendly call at the scheduled time to check in and see how the participant is doing.
Share How You’re Doing
Participants can simply check in, mention a concern, or request follow-up from Athens Home Care Solutions.
We Follow the Response Plan
When follow-up is needed—or a scheduled check-in isn’t completed—we follow the participant’s individualized response plan.
When a Little More Support Is Needed
Every participant has an individualized response plan so we know what to do when a check-in doesn't go as expected.
A Concern Comes Up
If a participant shares a non-emergency concern during their check-in, Athens Home Care Solutions can follow up according to their individualized response plan.
They'd Like Someone to Contact Them
Participants can request contact from Athens Home Care Solutions or ask that a designated family member or contact be notified.
The Daily Check-In Isn't Completed
If the participant doesn't answer their scheduled call, Athens Daily Connect will make another attempt. If the check-in still isn't completed, we follow the participant's established no-answer plan.
Designed Around Each Participant
Before service begins, we'll create an individualized response plan that identifies who should be contacted, what should happen if a check-in isn't completed, and how the participant would like different situations handled.

Stay Connected by Text
Authorized family members, caregivers, and other designated contacts can choose to receive text notifications related to Athens Daily Connect.
Text notifications are optional and are not required to participate in Athens Daily Connect.
